Fears are held for a teenager missing at a popular Gold Coast waterhole.

Emergency services are searching for the 19-year-old after he failed to resurface in the water at Killarney Glen waterhole in Beachmont.

It's believed he got into difficulties while swimming upstream and became stuck underwater about 11am on Friday.

A witness said the man's friends frantically sought help.

"As we rocked up there was a lady panicking on the phone and another two distraught young ladies and she said to us her boyfriend went under and didn't come back up," he told ABC radio.

The natural attraction is popular among Gold Coast hinterland locals, with a distinctive heart-shaped hole next to a waterfall.

However the waterhole, situated on land managed by the Australian Defence Force, has been in the spotlight in recent times following concerns about thrillseeker safety and traffic management.

Emergency services were called to the area in August and September this year when two women were injured in separate incidents.
