The Tasmanian government will conduct a health and lifestyle survey of 15,000 households to help formulate a plan to make the state the nation's healthiest by 2025.

The survey will include detailed questions around diet, physical activity and oral health, and test the understanding of information given by experts, public health spokeswoman Siobhan Harpur says.

Randomly selected participants will receive a letter in the mail inviting them to take part in the survey.
