Australian hospitals are failing stroke survivors with poor rehabilitation services, a new report says.

The Stroke Foundation has released the 2016 Rehabilitation Services Report, which says stroke patients in Australia are being denied the best care.

Key findings include that half of stroke patients are discharged from hospital without information about the cause of their stroke, only half are assessed for mental wellbeing and one in five are discharged without getting medication essential to preventing further strokes.

"We know what good care looks like but sadly too few patients are experiencing it," Stroke Foundation chief executive Sharon McGowan said.

The foundation is calling on federal and state governments to invest in quality stroke rehabilitation services across Australia.
