A man accused of hitting a woman with a 4WD and pinning her against another car in Sydney's southwest has appeared in court.

Police allege John Spiro Nichols, 46, drove onto a footpath in a landcruiser and pinned a 50-year-old woman against another 4WD at Busby late on Friday night.

It is also alleged he then reversed, before mounting a gutter and hitting a fence and stationary vehicle in a nearby street.

The woman sustained compound fractures to her legs, a fractured pelvis and lacerations to her head.

She was taken to hospital in a serious but stable condition and will undergo surgery.

Nichols has been charged with grievous bodily harm with intent and drive whilst disqualified.

He did not apply for bail in a brief appearance via audio visual link at Parramatta Bail Court on Saturday but Magistrate George Zdenkowski made a recommendation that injuries to his face be treated.

The matter has been listed for Campbelltown Local Court on Sept 21.
