A 69-year-old man has been killed after ploughing into a fence following a three-car prang in country NSW.

The man was fatally injured after the cars collided near Cowra on Thursday morning.

The other two drivers involved - men aged 27 and 41 - underwent mandatory blood and urine tests.

Later on Thursday, a woman died and a man suffered head injuries after a head-on car crash in northern NSW.

The 49-year-old woman was trapped inside her Toyota Hiace van for two hours after colliding with a four-wheel drive at Nimbin around 5.15pm.

She was airlifted to Gold Coast University Hospital, where she died from her injuries.

The 42-year-old man driving the second vehicle suffered head injuries and underwent mandatory testing.

His Toyota Landcruiser has been seized by police for mechanical examination.
