Police to be probed after death crash

NSW Police's Professional Standards Command have launched a critical incident investigation after a car being followed by a patrol car in Orange crashed and killed a rear-seat passenger.

The incident took place late on Tuesday night after officers spotted the car and began following it during a patrol.

A short time later the officers discovered the vehicle had crashed on Ophir Road, with a woman, who is yet to be formally identified, found dead while another passenger left uninjured, police say.

The driver and a third passenger fled the scene.
