A carjacker attacked a female doctor in a WA hospital car park then threatened to kill a teenage mother with a shovel as she held her baby in her backyard, police say.

The 41-year-old man, who is suspected of being affected by illicit drugs, forced his way into the doctor's car after she arrived for work at Bunbury Hospital on Monday morning, punching her in the face and leaving her with a broken nose and a black eye.

After stealing her vehicle, he abandoned it in College Grove on the town's outskirts and jumped fences into the backyard of a nearby house, where he startled the 19-year-old mother, then armed himself with a shovel and threatened to kill her.

When her 24-year-old partner ran outside and intervened, he was hit with the shovel and was treated in hospital for lacerations to his hands and head.

He was also bitten during the scuffle.

A neighbour held the man until police arrived.

The man is now in hospital under police guard while his fitness to be held in a cell is assessed.

A police spokesman said charges were pending and he could face court as soon as Tuesday.
