A woman has been airlifted to hospital in a serious condition after her car was rear-ended by a semi-trailer on a major road in Sydney's west, causing delays for motorists.

The woman, believed to be in her 40s, was hurt after her sedan was rear-ended by a truck on Northern Road, Cranebrook about 7.30am on Thursday.

The CareFlight helicopter made an emergency landing on the road with paramedics putting the woman in a coma before she was flown to Westmead Hospital.

The driver of the truck was taken to Nepean hospital by ambulance and Northern Road is closed in both directions at Fifth Avenue.
